Effective immediately, the Executive Director specifies the forms attached to this Notice under section 182 of the Securities Act.
Under NIN#98/33, the Executive Director specified amendments to a number of required forms, including Form 4 (“Uniformed Application for Registration”) and Form 36 (“Insider Report”). Under NIN#98/35, the Executive Director respecified Form 4 and Form 36, among others.
It has come to the attention of the Commission staff that both the Form 4 and the Form 36 specified in NIN#98/33, and respecified under NIN#98/35, contained technical errors in their published form. As a result, Form 4 and Form 36 are being specified and republished in their corrected form.
Registrants are encouraged to use the revised forms specified in this Notice as soon as copies are available. However, the Executive Director will accept a current version of a form specified in this Notice provided that the form is filed prior to September 30, 1998.
