Published Date: 1996-12-20
Effective Date: 1996-12-18

Effective Date

Section 152 of the Securities Rules requires an exchange issuer to file with the Commission a quarterly report prepared in accordance with Form 61. Effective February 1, 1997, National Instrument 13-101 requires exchange issuers that are reporting issuers in more than one Canadian jurisdiction to file Form 61 - Quarterly Reports electronically under SEDAR. Effective July 1, 1997, exchange issuers that are reporting issuers in British Columbia only are required to file Form 61 - Quarterly Reports electronically under SEDAR.

How to File

Form 61 - Quarterly Reports are filed under Category of Filing: Continuous Disclosure and Filing Type: Interim Financial Statements or Annual Financial Statements. Schedule A (Financial Statements) is filed under Document Type: Interim Financial Statements or Annual Financial Statements. Schedule B (Supplementary Information) and Schedule C (Management Discussion) are filed under Document Type: Form 61(BC).

Meeting the Form Requirements

Form 61 consists of three parts, instructions to schedules A, B and C, issuer details and a certificate. To comply with National Instrument 13-101 it is not necessary to reproduce the instructions that are set out in Form 61. A cover page to the schedules titled Form 61 that includes the issuer details and certificate is all that is required to meet the Form 61 requirements. The form of certificate should be amended so as to refer to one or two of the three schedules required to complete the Quarterly Report.
