This Personal Information Form and Authorization of Indirect Collection, Use and Disclosure of Personal Information (the "Form") is to be completed by every individual who is required to do so by the Executive Director in connection with an OTC Reporting Issuer as set out in section 2.2 of BC Policy 13-601 Required Forms.   Where an individual has submitted a personal information form (an "Exchange Form") to the Toronto Stock Exchange or the TSX Venture Exchange and the information has not changed, the Exchange Form may be delivered in lieu of this Form if the certificate and consent of this Form is completed and attached to the Exchange Form.
